Using our online form
The quickest way to contact us about your healthcare needs is by using our online form.
Use our online form to send us requests for:
- Medical care, such as a GP appointment
- Sick notes
- Admin queries
- Referrals or follow-up questions.
Our online form is provided by a company called Anima. For questions and support, visit our Anima Frequently Asked Questions webpage.
Using the NHS App or NHS website
Using the NHS App or the NHS website, you can:
- Send us a medical or admin request with our online form (NHS App only)
- Order repeat prescriptions
- Check or cancel your appointments
- Check test results
- View your health record, such as GP/hospital letters and your vaccinations
- Get online health advice from NHS 111
- Find other local healthcare services.
Using the Medicine Order Line
You can use the Medicine Order Line to order medications and repeat prescriptions, either:
- Via telephone on 01246 588860 between 8am and 4pm (Monday to Friday – not including Bank Holidays)
- Using the Derbyshire Medicine Order Line online form.
Using SystmOnline
With SystmOnline, there are other tasks you can do online such as:
- Order repeat prescriptions
- View your test results
- See parts of your health record
- Check or cancel appointments
- See your vaccination record.
Using Airmid
Through Airmid, you can:
- View upcoming appointments and add appointment reminders to your calendar
- Check-in for your appointments when you arrive at our surgery
- Change your nominated pharmacy
- Request medication
- View your medical records including consultation notes, medication history, vaccination history, allergy/drug sensitivities, procedure history, correspondences e.g. referral/discharge letters, and test results
- Import and chart health data from your personal device such as Apple Health and Google Fit.
You can log in to Airmid using your SystmOnline username and password if you have them. You can also use your NHS account login.
Setting up an NHS account
To set up your NHS account for the first time, you will need to set up an NHS login. Read about how to set up NHS login (nhs.uk).
Once you have set up your NHS login, you can log in to your NHS account or download the NHS App onto your phone.
To set up SystmOnline for the first time:
- Visit the surgery with photo ID so we can confirm your identity
- We will give you a username and password
- Go to SystmOnline or use the app to register.
You can log in to Airmid using your SystmOnline username and password or using your NHS account login.
- Set up your NHS login using the NHS website

Following initial setup, you can choose to authenticate your login with biometrics such as face ID or touch ID to ensure quick and secure access.
